Care and Support Coordinator

The employer is a charitable organisation which runs various services for vulnerable people for more than 40 years. This vacancy is for their supported living service in North West London. Supported living enables a person with a disability or an older person to get the support he or she needs to live in and manage his or her own home. The employer’s commitment is to working in an anti-discriminatory way and a positive view of a range of vulnerable people, including people with physical and/or learning disabilities, people with mental health, young people, older people, sensory impairment, autism spectrum disorder (ASD).

They have different properties with different levels of support to suit their customer’s needs.

Main purpose of this job:

To engage, enable and empower customers to support them to live the life they choose

Responsibilities /Overview of Role:

  • Provide high quality care and support services to customers including personal and hygiene care where required
  • Coordinating customers care packages
  • Linking customers with opportunities in the community
  • Partnership working and networking with other stakeholders
  • Support customers to manage their finances as per policy where required
  • Act as a role model at all times
  • Provide advice, support and guidance to support workers
  • Provide supervision to support worker, volunteers and apprentices
  • Record keeping

Required Education Level social care qualified
Qualifications Preferably social care qualified equivalent to NVQ Level 2 or 3 in Health and Social Care / desirable: experience working with vulnerable people and driving licence
